This training will provide an in-depth look at the Colorado Department of Public Health and Environment (CDPHE) reporting requirements for Occurrences. We review Chapter 2, Section 4.2 - Occurrence Reporting; Chapter 26, Section 6.10 - Agency Reporting Requirements and the Health Facilities and Emergency Medical Services Division Occurrence Reporting Manual. Each of the 14 Occurrence types are reviewed along with example reporting scenarios. We also discuss tools agencies can use to ensure staff are properly trained, as well as tools for agency reporting and investigation process.

This training satisfies 3 CLOCK HOURS towards the annual required training for Class A Administrators & Alternate Administrators and Class B Agency Managers & Back-Up Agency Managers. A completion certificate will be available for download upon completion of the course. What is covered in this course? 

This course is divided up into 4 parts. The first part is the Digital Downloads section. In this Digital Downloads section there are helpful guides, documents and policies that you can utilize for your agency related to Occurrence Reporting. In addition, there are 3 Modules of content in this course providing detailed information about Occurrence Reporting. 

In order to complete the course, you must complete each of the modules along with a Quiz at the end of each module. This online course is available 24/7 and you can start, pause and return to where you left in the course at any time.

Abuse & Neglect Prevention and Agency Occurrence Reporting Requirements - Course Outline - 3 Clock Hours - This document provides the course outline on the content covered. Agency Occurrence Reporting Policy - This document is an example policy for an agency reporting policy as required by CDPHE. It is customizable to fit the needs of any agency.

Health Facilities Organizational Chart - Updated August 2019  - This organizational chart shows the different branches and includes names and titles. 

Agency Occurrence Log - This document provides a customizable log that each agency can use to track Occurrences as they are reported throughout the year. 

Deaths Reportable to Local Coroner Policy - This example policy is required by CDPHE from Chapter 2, Section 4.2 for each agency to have a policy regarding deaths reportable to the local coroner. 

Abuse & Neglect Prevention and Agency Occurrence Reporting Requirements - Staff Training - This document is an example policy and training sheet that can be used when providing training to staff. It is an individualized document that can be signed by each agency staff member to show that each staff member has been training.

Agency Occurrence Investigation Checklist & Form - This customizable document provides a checklist and investigation form for an agency to conduct an Occurrence investigation. The form ensures that your agency are following the appropriate steps during an investigation and the checklist ensures you have included everything in your Occurrence file once the investigation has bee completed.
