Avoid the #1 Tag for Home Care in 2023!

So what's the most issued deficiency for home care companies in Colorado?

#1 - Incomplete Consumer Records

Have you taken a peek into you client files lately? Now may be a perfect time to do a quarterly client record audit.

As part of your annual evaluation policy, each home care agency is required to evaluate consumer records at least QUARTERLY. (Chapter 26, Section 6.3F, 7.1D(6))

Today, let's take a look at what is required in each consumer file so you can do a proper audit!

Here's the documentation required in each Client File...

Make sure you know what should go in each Client File

Signing up a new client can be exciting! But, making sure your collect all the correct paperwork can be time consuming.

Do you have admission packets printed and ready to go? Or, does your EMR allow you to complete all paperwork electronically?

CDPHE has a list of documents that they will expect to see when they review a client file. Also, there are likely some forms that you want to be in each client file.

If you need a list of Class A & B client file documents, scroll down to the BLUE BUTTON! That's my gift to you today.

Here's regulation 7.1D(6) in Chapter 26

(6) Appropriate qualified individuals representing the programs and services offered by the HCA shall evaluate the HCA's consumer records on an ongoing basis, but no less than quarterly.

(a) The evaluation shall include a review of sample active and closed consumer records to ensure that HCA policies are followed in providing services, both directly and under arrangement, and to assure that the quality of service is satisfactory and appropriate. The review shall consist of a representative sample of all home care services provided by the HCA.

What is a 'sample' number of consumer records? Good question and it is open to interpretation. A good rule of thumb is to audit between 5% to 10% of your client files per quarter. This will give you a good idea of compliance to meet this regulation. Then, if you find chronic problems, you can continue onto more files.

Auditing client files can be about as fun as a root canal. But, a proactive audit on a quarterly basis can help identify big issues prior to survey showing up.
